What is Self-Directed Care?

Self-directed home care services are a type of long-term care that puts you in charge. You are the employer of your caregivers—
you choose who will assist you and what tasks they do.

But you don’t have to handle everything alone. Consumer Direct Care Network can support you with things like sending in forms, keeping track of your hours, and paying caregivers.

If you need help at home, self-direction may be an option for you.

Consumer Direct Care Network Colorado

Consumer Direct Care Network Colorado (CDCO) helps support home care for individuals who receive Medicaid services and are eligible for a Home and Community-Based Services (HCBS) waiver or the Veteran Directed Care program.

Consumer Directed Attendant Support Services (CDASS) is a Medicaid service option under the HCBS program. Through CDASS, you can manage your own care needs rather than going through a home health agency:

  • Your Case Manager works with you to determine your need and eligibility.
  • You hire, train, and schedule your attendants.
  • You have support with administrative, employment, and payroll-related tasks.

The Veteran Directed Care program offers enrolled Veterans more choice, control, and flexibility over the services they receive. The Veteran Directed Care program includes participant-directed services as an alternative to the VA’s agency-based care options.

To see if there is a Veteran Directed Care program in your area, contact your local VA Medical Center or click here for a list of active programs.

Our programs are here to support you in the comfort and safety of your home.
